Small Cell Lung Cancer (SCLC) KOL Interview - Spain
October 20, 2026
This interview with a Spain-based key opinion leader (KOL) offers insights into current treatment practices, prescribing behaviors, and late-phase pipeline therapies in Small Cell Lung Cancer (SCLC). The discussion covers disease staging, diagnostic approaches, first- and second-line treatment selection, and the evolving role of immunotherapy and antibody-drug conjugates (ADCs) in the SCLC treatment landscape. Additional topics include patient eligibility for emerging therapies, treatment sequencing in the relapsed/refractory setting, unmet needs, and the potential impact of novel pipeline agents.
The standard of care for extensive-stage SCLC in Spain relies on carboplatin/etoposide plus atezolizumab as first-line therapy, as durvalumab remains unapproved in the country. The expert noted that both tarlatamab and lurbinectedin are not yet approved in Spain, with topotecan remaining the standard second-line option. The KOL expressed a strong preference for tarlatamab over lurbinectedin based on DeLLphi trial data, and anticipated lurbinectedin's potential future role as a maintenance or first-line agent. Pipeline ADCs were discussed with cautious optimism tempered by toxicity concerns, while DLL3-targeted BiTE therapies were viewed as more promising. Key unmet needs include developing more durable, less toxic therapies and advancing the most effective agents to the first-line setting.
This interview was conducted on April 20, 2026.
